Fees, Uniforms, and Payment
The basic fee per athlete is $250 for the season, and includes include your uniform/sweatsuit, AAU membership, and all meet and team fees. At the conclusion of the season, you may turn in your uniform for a $50 refund. Please note: Fees do not include additional expenses associated with Regional and Qualifying Meets, and do not cover travel expenses. |

Submitting this form online registers you for the team, however, your registration is not complete!
New athletes must also provide a copy of their birth certificate or passport as proof of age. All athletes must provide a copy of their first-semester report card.
In addition, you must print, complete, and sign the following forms:
- Waiver, Release, and Statement of Physical Condition
- Athletes Contract
- Parents Code of Conduct

Once they are completed, please bring them, along with a check for the total amount due as indicated above and a copy of your first-semester report card and copy of your birth certificate, to the registration session from 6:00 to 7:30 pm on March 16 or 17, 2010, at Northwest High School.
